Step 1: Gather the episode data before opening ChatGPT
Good discharge summaries require accurate data. Before you start, pull up the patient's chart and note:
- Initial evaluation date and presenting diagnosis
- Baseline functional status (initial ROM, strength, pain, functional scores)
- Total visits and duration of care
- Main interventions used
- Final status (current ROM, strength, pain, functional scores)
- Goals from the initial plan of care — which were met, partially met, or not met
- Discharge home program plan
Having this in front of you takes 2–3 minutes but makes the AI output dramatically more accurate.
