By the end of this guide, you'll have ChatGPT configured as a personalized documentation assistant that knows your clinical style, your common diagnoses, and your preferred note format. Instead of starting every session from a generic prompt, it'll already understand that you're a PT specializing in orthopedics and that you like concise, functional goal-oriented language.

What you'll need

  • A ChatGPT account — free at chat.openai.com (Custom Instructions work on free; ChatGPT Plus at $20/month unlocks GPT-4 for higher quality)
  • 30 minutes to set up your custom instructions
  • Time needed: 30-45 minutes setup, then 2-3 minutes per note
  • Cost: Free (or $20/mo for ChatGPT Plus)

How-To Guide: Your Personal Clinical Documentation Assistant

Step 1: Open ChatGPT and go to Custom Instructions

  1. Go to chat.openai.com and log in
  2. Click your profile picture or icon in the bottom-left corner
  3. Click "Custom Instructions" from the menu
  4. You'll see two text boxes: "What would you like ChatGPT to know about you?" and "How would you like ChatGPT to respond?"

What you should see: Two large text boxes for entering your persistent preferences.

Troubleshooting: If you don't see "Custom Instructions," your account may need to enable it in Settings → General.
